Job Summary
Develop a plan of care through physician orders, client input, and nursing assessment to include medical interventions and measurable goals. Provide skilled nursing care in patients' homes using skills such as phone triage, wound care, infusions, and catheter care while educating clients and training primary caregivers. Coordinate the continuum of hands-on client care through documentation and timely communication with physicians and other caregivers. Schedule part-time visits Monday through Friday with an on-call rotation in Jasper, Nauvoo, Carbon Hill, Haleyville, Winfield, and Fayette. Requires an active RN license, valid CPR or BLS, and a valid driver's license.
Required Qualifications
- An active RN License in the state of application
- Valid CPR or BLS
- Valid Drivers License
- Must comply with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing, subject to exemptions for medical or religious reasons as appropriate
Desired Qualifications
- Medicare Skilled Nursing experience
- Basic understanding of Oasis
- 1-year RN experience in a health care setting
