Home Health Registered Nurse Part Time

Deliver one-on-one skilled nursing care in the home, conducting comprehensive assessments to evaluate physical, emotional, and environmental needs. Provide interventions including wound care, infusions, medication management, and chronic disease management while educating patients and caregivers on symptom and daily care management. Monitor patient progress, communicate with physicians and interdisciplinary teams to adjust plans, and coordinate care across providers to ensure continuity and prevent hospitalizations. Complete timely, accurate documentation reflecting the full clinical picture. Serve as the primary clinical partner for patients and families in Garrison, Aitkin, Deerwood, and Onamia on a PRN basis Monday through Friday with a seven-day on-call rotation.
