Home Health Registered Nurse

Provide comprehensive skilled nursing care to patients in their homes, managing a caseload from admission to discharge with autonomy and collaboration across a multidisciplinary team. Perform initial evaluations and ongoing OASIS assessments, deliver skilled nursing procedures, phlebotomy and wound care, educate patients and families on home nursing procedures and self-care, develop and revise individualized care plans, coordinate with physicians, therapists, social workers and other providers, and supervise home health aides and paraprofessionals. Maintains timely documentation, participates in quality improvement, and travels daily within the Jackson, MI service area to deliver care in diverse home environments. Requires RN licensure, a minimum of 1 year clinical experience, and willingness to travel daily to patient homes. Preferred BSN and home health experience; OASIS certification or willingness to obtain; strong clinical judgment, teaching skills, and ability to work independently while coordinating with a multidisciplinary team.
