Job Summary
Provides coordinated skilled nursing care to home-based patients across all ages, collaborating with an interdisciplinary team to achieve patient goals, document with OASIS and visit records, and supervise home health aides/LPNs/LVNs while participating in on-call rotations. Applies physician orders and ensures timely communication of significant findings to the Clinical Manager and physician, with focus on maintaining high-quality, continuous care in the patient’s home setting.
