Job Summary
Assess and plan care for home health patients per CMS guidelines, creating OASIS documentation and coordinating re-visits. Manage therapist teams to ensure patients meet goals and achieve safe community discharge. Execute skilled IV infusions, phlebotomy, wound care, and NGT/GT management while teaching disease processes. Supervise LVNs and CHHAs, attend case conferences, and maintain flexibility for Bay Area travel. Requires one year acute/Med-Surg and one year home health experience, California RN license, and CPR certification. Full-time staff receive $124K–$165K plus mileage, parking, and comprehensive benefits including 401(k) match.
Required Qualifications
- 1 year full time experience in Med/Surg or acute setting
- 1 year minimum home health experience
- RN unrestricted license in California
- CPR
- California Driver's License
- valid auto insurance
- Skilled in IV infusion
- phlebotomy
- wound care including wound vacs
- NGT/GT
- teaching and management of all disease processes
- Able to manage an independent schedule
- supervise LVN's & CHHA's
- process excellent communicate skills
- integrity and loyalty required
- Maintain flexibility with the challenges of Bay Area car travel from home to home
- Experienced in home health software and other electronic communication
- Critical thinking skills
