Job Summary
Conduct comprehensive assessments of terminal disease impacts on patients' physical, functional, psychosocial, and environmental needs. Apply admission and re-certification criteria to establish care levels and eligibility. Implement and document plans of care while monitoring cases for third-party payor compliance. Provide clinical direction to aides and LPNs/LVNs, prepare progress notes, and inform physicians of patient changes. Educate patients and families on disease processes, self-care, nutrition, and end-of-life care. Determine service frequency based on acuity and assess caregiver ability. Work independently in Austin, TX, as a PRN pay-per-visit RN at $50 per visit with competitive pay and training opportunities.
Required Qualifications
- Current RN license and good standing with the state nursing board

- Knowledge or experience utilizing Homecare Homebase EMR
- Experience in home health or related healthcare setting
