Job Summary
Deliver individualized, one-on-one nursing care in the home, conducting comprehensive assessments to evaluate physical, emotional, and environmental needs. Provide skilled interventions including wound care, infusions, and medication management while educating patients and caregivers on symptom management. Monitor patient progress, communicate with physicians and the interdisciplinary team to adjust plans, and coordinate care across providers to ensure continuity. Complete timely, accurate documentation reflecting the full clinical picture. Maintain a Monday through Friday schedule with shared on-call rotation in the Sylacauga, Lincoln, Talladega, and surrounding territories.
Required Qualifications
- Active Registered Nurse (RN) license in the state of practice
- At least one (1) year of clinical nursing experience
- Patient-focused mindset
- Organized mindset
- Motivated by delivering high-quality care
- Current driver's license
- Reliable transportation
- Auto insurance
- CPR certification (or ability to obtain certification)
- Compliance with all health-related requirements in all relevant jurisdictions, including required vaccinations and testing
