Provide skilled care tailored to each patient's needs to facilitate their journey toward independence. Conduct thorough initial assessments to develop personalized care plans in collaboration with physicians and the interdisciplinary team. Educate patients and their families on ongoing care strategies and lifestyle improvements while coordinating and administering skilled treatments aimed at enhancing patient well-being and overall health. Requires a valid registered nursing license, home health experience, and one year of registered nursing experience. Includes an Associate degree in nursing.
