Job Summary
Provide skilled care tailored to each patient's needs to facilitate their journey toward independence. Conduct thorough initial assessments to develop personalized care plans in collaboration with physicians and the interdisciplinary team. Educate patients and their families on ongoing care strategies and lifestyle improvements. Coordinate and administer skilled treatments aimed at enhancing patient well-being and overall health. This contingent role in the Detroit Area requires a valid registered nursing license, one year of RN experience, an associate degree, and home health experience.
