Job Summary
Home Health Registered Nurse (RN) provides patient-focused care at patients’ homes, supports recovery to enable home-based living, educates patients and families on disease management, and case-manages to oversee an interdisciplinary team. Requires an active RN license in the work state, at least two years of nursing experience (home health preferred), a valid driver’s license, and current CPR/BCLS certification.
