Home Health Registered Nurse

Provide skilled nursing care to patients onsite in their homes, including administering medications, performing wound care, and conducting patient assessments. Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals while monitoring conditions and reporting changes. Educate patients and families on disease management and treatment options, maintain accurate care records, and oversee the work of Home Health Aides. Ensure all patient care progress is reported to the physician and Clinical Manager. Requires current RN licensure, med surg or ICU experience, and a valid driver's license for this patient-facing role.
