Home Health Registered Nurse

CenterWell Home Health is hiring a Home Health Registered Nurse to provide skilled, one-on-one nursing care in patients’ homes. Responsibilities include administering medications, wound care, and patient assessments; developing and implementing individualized care plans with physicians and other healthcare professionals; monitoring patient conditions and reporting changes; educating patients and families on disease management and treatment options; maintaining accurate care records and coordinating with the care team; supervising Home Health Aides and ensuring documentation is completed; and collaborating with physicians and clinicians to support patient recovery and independence. This role is full-time (40 hours) with home-based visits and requires a valid driver’s license and driver-insurance, plus TB and driver-safety program compliance.
