Job Summary
Provide skilled nursing care onsite in patients' homes, including administering medications, performing wound care, and conducting assessments. Develop and implement individualized care plans while monitoring conditions and reporting changes to physicians and the Clinical Manager. Educate patients and families on disease management and treatment options. Maintain accurate care records, coordinate with healthcare professionals, and oversee Home Health Aides' documentation completion. Requires current RN licensure, med surg/ICU/ER experience, and a valid driver's license. Full-time 40-hour schedule with annual base pay ranging from $77,200 to $106,200.
Required Qualifications
