Job Summary
Provide skilled nursing care onsite in patients' homes, including administering medications, performing wound care, and conducting assessments. Develop and implement individualized care plans while monitoring patient conditions and reporting changes to physicians and the Clinical Manager. Educate patients and families on disease management and treatment options, and maintain accurate care records. Oversee Home Health Aides and ensure appropriate documentation completion. Requires current RN licensure, med surg/ICU/ER experience, and a valid driver's license with auto insurance. Full-time 40-hour schedule with pay ranging from $77,200 to $106,200 annually.
Required Qualifications
- Med surg, ICU, ER, or acute experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Valid driver's license
- Auto insurance
- Reliable transportation
- TB screening
