Home Health Registered Nurse

Provide skilled nursing care to patients onsite in their homes, including administering medications, performing wound care, and conducting patient assessments. Develop and implement individualized care plans in collaboration with physicians, monitor patient conditions, and report changes to the physician and Clinical Manager. Educate patients and families on disease management and treatment options while maintaining accurate records. Oversee Home Health Aides and ensure appropriate completion of documentation. Requires current and unrestricted RN licensure, med surg/ICU/ER experience, and a valid driver's license. Full-time 40-hour weekly schedule with pay per visit ranging from $54.00 to $75.00.
