Home Health Registered Nurse

Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting assessments. Develop and implement individualized care plans in collaboration with physicians, monitor patient conditions, and report progress to the Clinical Manager. Educate patients and families on disease management and treatment options while maintaining accurate care records. Oversee Home Health Aides and ensure appropriate documentation completion. Requires current and unrestricted RN licensure, CPR certification, and valid driver's license with auto insurance. Full-time 40-hour weekly schedule with pay per visit ranging from $44.55 to $61.28.
