Job Summary
Provide skilled nursing care onsite in patient homes, including administering medications, performing wound care, and conducting assessments. Develop and implement individualized care plans while monitoring conditions and reporting changes to physicians and the Clinical Manager. Educate patients and families on disease management and treatment options. Maintain accurate records and coordinate with healthcare professionals. Oversee Home Health Aides and monitor their documentation completion. Requires current RN licensure, med surg/ICU/ER experience, and a valid driver's license. Pay ranges from $49 to $69 per visit or $77,200 to $106,200 annually.
Required Qualifications
- Diploma, Associate or Bachelor's Degree in Nursing
- Med surg, ICU, ER, or acute experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Valid driver's license
