Home Health Registered Nurse

Provide skilled nursing care onsite in patients' homes, including administering medications, performing wound care, and conducting assessments. Develop and implement individualized care plans with physicians and other healthcare professionals. Monitor patient conditions, report changes, and educate families on disease management and treatment options. Maintain accurate records, coordinate with the care team, and oversee Home Health Aides' documentation completion. This role reports to the Clinical Manager and involves managing care plans to support patient recovery and independence. Candidates must have med surg, ICU, or ER experience, current RN licensure, and a valid driver's license. Full-time 40-hour weekly schedule with pay per visit ranging from $49.00 to $69.00.
