Job Summary
Provide skilled nursing care to patients onsite in their homes, including administering medications, performing wound care, and conducting patient assessments. Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals. Monitor patients' conditions, report changes, and educate families on disease management and treatment options. Maintain accurate records, coordinate with professionals, and oversee Home Health Aides' documentation completion. Report care progress to the physician and Clinical Manager. Requires med surg, ICU, or ER experience; current RN licensure, CPR certification, and valid driver's license. Full-time 40-hour schedule with pay per visit/unit rate.
Required Qualifications
- Med surg, ICU, ER, or acute experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Valid driver's license

- Reliable transportation
