Job Summary
Provide skilled nursing care in patients' homes, including medications administration, wound care, and patient assessments; develop and implement individualized care plans with physicians and healthcare professionals; monitor patient conditions and educate patients and families on disease management and treatment options; oversee Home Health Aides and ensure documentation completion; drive to patient homes as part of home health care services; 40 hours per week with pay range $69,500–$95,600 annually; requires RN licensure, CPR, and valid driver's license; Driver and TB screenings per Humana CenterWell policy.
Required Qualifications
- Med surg, ICU, ER, or acute experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Valid driver's license, auto insurance and reliable transportation
