Job Summary
CenterWell Home Health is seeking a Home Health Registered Nurse to provide one-on-one skilled nursing care in patients' homes. Responsibilities include administering medications, wound care, and patient assessments; developing and implementing individualized care plans with physicians and healthcare professionals; monitoring patient conditions and reporting changes; educating patients and families on disease management, medications, and treatment options; maintaining accurate care records and coordinating with other professionals; overseeing Home Health Aides and ensuring proper documentation. Eligible candidates must hold an active RN license, CPR certification, and have reliable transportation with a valid driver’s license.
Required Qualifications
- Med surg, ICU, ER, or acute experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Valid driver's license, auto insurance and reliable transportation
- One year nursing experience
