Job Summary
Home Health Registered Nurse providing one-on-one skilled nursing care in patients' homes; administer medications, perform wound care, and conduct patient assessments; develop and implement individualized care plans with physicians and clinicians; monitor conditions and report changes; educate patients and families on disease management, medications, and treatment options; maintain records, coordinate with healthcare professionals, and oversee Home Health Aides to ensure proper documentation and care; requires collaboration with a clinical team and driving to patient homes; TB screening and driver-safety program participation are part of the role; 40 hours per week with compensation reflecting base pay and location.
Required Qualifications
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Med surg, ICU, ER, or acute experience
- Valid driver's license, auto insurance and reliable transportation
