Job Summary
Provide skilled nursing care to patients in their homes, including administering medications, wound care, and patient assessments. Develop and implement individualized care plans in collaboration with physicians and healthcare professionals; monitor patient conditions and report changes. Educate patients and families on disease management, medications, and treatment options. Maintain thorough documentation and coordinate with other healthcare professionals. Oversee Home Health Aides and ensure timely completion of documentation. This role supports direct, in-home patient care within the CenterWell Home Health and Humana family.
Required Qualifications
- Med surg, ICU, ER, or acute experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Valid driver's license, auto insurance and reliable transportation
