Job Summary
CenterWell Home Health is seeking a Home Health Registered Nurse to provide one-on-one skilled nursing care in patients' homes. Responsibilities include administering medications, wound care, patient assessments, developing and implementing individualized care plans with physicians and healthcare professionals, monitoring patient conditions, educating patients and families, maintaining accurate care records, coordinating with healthcare teams, and supervising Home Health Aides. This role requires valid RN licensure, CPR certification, and a valid driver’s license with reliable transportation; one year of nursing experience and home health experience are preferred.
Required Qualifications
- Med surg, ICU, ER, or acute experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Valid driver's license, auto insurance and reliable transportation
