Job Summary
Provide skilled nursing care in patients' homes, including administering medications, wound care, and patient assessments; develop and implement individualized care plans with physicians and healthcare professionals; monitor patient conditions and report changes; educate patients and families on disease management, medications, and treatment options; maintain accurate records and coordinate with healthcare teams; oversee Home Health Aides and ensure documentation completion; role is 40 hours/week in home-based, in-person settings.
Required Qualifications
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Med surg, ICU, ER, or acute experience
- Valid driver's license, auto insurance and reliable transportation
