Job Summary
Home Health Registered Nurse providing one-on-one skilled nursing care in patients' homes, including medication administration, wound care, and comprehensive patient assessments; develop and implement individualized care plans with physicians and healthcare professionals; monitor patient conditions and communicate changes to the physician and Clinical Manager; educate patients and families on disease management, medications, and treatment options; oversee Home Health Aide documentation and ensure care coordination across the care team.
Required Qualifications
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Med surg, ICU, ER, or acute experience
- Experience collaborating with a team of healthcare professionals
- Valid driver's license, auto insurance and reliable transportation
