Job Summary
Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments. Develop and implement individualized care plans while monitoring conditions and reporting changes to physicians and the Clinical Manager. Educate patients and families on disease management and treatment options, and maintain accurate care records. Oversee Home Health Aides and ensure appropriate documentation completion. Requires current and unrestricted RN licensure, CPR certification, and a valid driver's license with auto insurance. Pay range is $84,900 - $116,800 annually for 40 hours per week.
Required Qualifications
- Med surg, ICU, ER, or acute experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
