Job Summary
Provide skilled nursing care to patients in their homes, including administering medications, performing wound care, and conducting patient assessments. Develop and implement individualized care plans in collaboration with physicians, monitor patient conditions, and report changes to the Clinical Manager. Educate patients and families on disease management and treatment options while maintaining accurate care records. Oversee Home Health Aides and ensure appropriate documentation completion. Requires current and unrestricted Registered Nurse licensure, Med surg/ICU/ER experience, and a valid driver's license with auto insurance. Full-time 40-hour weekly schedule with a pay range of $84,900 - $116,800 annually.
