Job Summary
Provide skilled nursing care to patients in their homes, including administering medications, wound care, and comprehensive patient assessments. Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals, monitor patient conditions, and report changes. Educate patients and families on disease management, medications, and treatment options. Maintain accurate documentation and coordinate with other healthcare professionals. Oversee Home Health Aides to ensure proper documentation and care delivery. This role requires driving to patient homes in Gastonia, NC, and supports a multidisciplinary team to help patients recover and regain independence.
Required Qualifications
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Med surg, ICU, ER, or acute experience
- Experience collaborating with a team of healthcare professionals
- Valid driver's license, auto insurance and reliable transportation
