Job Summary
Provide skilled nursing care to patients in their homes, including administering medications, wound care, and patient assessments; develop and implement individualized care plans in collaboration with physicians; educate patients and families on disease management and treatment options; monitor patient conditions and coordinate with healthcare professionals; oversee Home Health Aides and ensure documentation is completed accurately; work onsite in patients' homes and report progress to the physician and Clinical Manager.
Required Qualifications
- Med surg, ICU, ER, or acute experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Valid driver's license, auto insurance and reliable transportation

- TB screening program participation
