Job Summary
CenterWell Home Health is seeking a Home Health Registered Nurse Case Manager to provide skilled nursing care in patients' homes, develop and implement individualized care plans in collaboration with physicians and other healthcare professionals, monitor patients' conditions, educate patients and families on disease management and treatment options, maintain accurate care documentation, coordinate with the healthcare team, report patient progress to the physician and Clinical Manager, and oversee Home Health Aides.
Required Qualifications
- Med surg, ICU, ER, or acute experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Experience collaborating with a team of healthcare professionals
- Valid driver's license, auto insurance and reliable transportation
