Home Health Registered Nurse Case Manager

Provide skilled nursing care to patients onsite in their homes, including administering medications, performing wound care, and conducting patient assessments. Develop and implement individualized care plans in collaboration with physicians and other healthcare professionals while monitoring conditions and reporting changes. Educate patients and families on disease management and treatment options, maintain accurate records, and oversee the work of Home Health Aides to ensure appropriate documentation completion. Report patient care progress to the physician and Clinical Manager. Requires current and unrestricted Registered Nurse licensure, med surg/ICU/ER experience, and a valid driver's license. This role supports patients managing chronic conditions or recovering from injury, illness, or surgery within CenterWell's integrated home care network.
