Job Summary
Provide skilled nursing care to patients in their homes, including administering medications, wound care, and patient assessments; develop and implement individualized care plans in collaboration with physicians and other healthcare professionals; monitor patients' conditions and report changes; educate patients and families on disease management, medications, and treatment options; maintain accurate records and coordinate with other healthcare professionals; oversee Home Health Aides and ensure documentation compliance; role reports to Clinical Manager within CenterWell Home Health.
Required Qualifications
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Med surg, ICU, ER, or acute experience
- Valid driver's license, auto insurance and reliable transportation
