Job Summary
Deliver skilled nursing care in patients' homes in accordance with physician orders and individualized plan of care; collaborate with the interdisciplinary care team to develop, implement, and update personalized care plans; administer nursing treatments and manage clinical activities to support optimal health outcomes; continuously assess patient progress and communicate findings with the care team, patient, family, physician, and case manager; additional duties as needed. Utilize Roger app for HIPAA-compliant, AI-assisted documentation and WellSky/Kinnser electronic charting to reduce charting time while maintaining adherence to OASIS requirements. Coverage area includes Arlington/Grand Prairie/Mansfield; remote workforce options available per role requirements, with 24/7 support and ongoing training.
Required Qualifications
- Active RN License (TX/Compact)
- Active CPR Certificate
- Home Health Experience – Prior experience in geriatric home health care
- Self-Starter with Organizational Skill
- Compassionate Care Approach
- Valid Driver’s License
