Job Summary
Deliver comprehensive skilled nursing care to patients in their homes, managing caseloads from admission through discharge while conducting assessments and developing individualized care plans. Coordinate services with physicians and interdisciplinary teams to support recovery, chronic disease management, and overall health goals. Provide clinical judgment and critical thinking in diverse home, assisted living, and nursing facility environments with minimal supervision. Monday to Friday, 8 a.m. to 5 p.m. with daily travel required within the Grand Rapids service area. $76,000—$84,000 USD. BSN preferred with OASIS certification.
Required Qualifications
- Graduate of an accredited Nursing School: ASN or BSN
- Current State License as a Registered Nurse
- Minimum 1 year of experience as a licensed RN in a clinical care setting
- Reliable transportation
- Valid driver's license
- Valid automobile insurance coverage
- Willingness to travel daily for home visits within service area
Desired Qualifications
- Bachelor of Science in Nursing (BSN) preferred
- Experience in home health environment preferred
- OASIS certification or willingness to obtain preferred
