Provide direct physical therapy services in home health under the physical therapist's plan of care; educate patients and caregivers on safe use of devices and carry over rehabilitation techniques; update patient and caregiver on plan of care; document progress and discharge plans; collaborate with interdisciplinary team; maintain confidentiality and complete timely documentation; ensure consistent attendance and credential submission; CPR required; licensure in applicable state.
Required Qualifications
- Graduation from a Physical Therapist Assistant education program from an accredited university/program
- Current, unrestricted state license as a Physical Therapy Assistant in the state of practice
- Valid CPR
