Home Health PTA Physical Therapist Assistant Full Time

Provide direct physical therapy services, treatments, and procedures in the home health plan of care as directed by the physician. Collaborate with interdisciplinary team members to update patients and caregivers on rehabilitation progress, while reporting observations of condition changes to supervisors or physicians. Educate patients, family members, and caregivers on safe use of assistive devices and adaptation techniques for recovery. Document clinical progress notes and complete daily reports timely to ensure continuity of care and agency schedules. Maintain consistent attendance and comply with professional standards and confidentiality requirements. Full-time Monday through Friday visits in Alachua County.
