Job Summary
Provide direct physical therapy, treatments, and interventions as directed by the physician in the home health plan of care. Communicate regularly with patients and caregivers regarding the plan of care, report observations of the patient's condition to supervisors or the physician, and educate families on safe use of assistive devices and rehabilitation techniques. Collaborate with interdisciplinary team members to discuss the patient plan of care, plan for client discharge, and prepare discharge instructions. Record clinical progress notes and complete daily reports to ensure continuity of care and agency operating schedules. This full-time role in Marion County requires a Physical Therapist Assistant with an accredited degree and current state license. Required Qualifications
- Graduation from a Physical Therapist Assistant education program from an accredited university/program
- Current, unrestricted state license as a Physical Therapy Assistant in the state of practice
- Valid CPR
- Required vaccinations and testing
