Job Summary
As the Home Health Physical Therapist Assistant, you will administer the physical therapy treatment plan, under the supervision of a Physical Therapist, to improve patients' strength, mobility and independence in their homes; you will document progress in patient records, participate in care coordination and discharge planning, and serve as a resource to other healthcare personnel while supporting rehab and functional improvement.
Required Qualifications
- Current and unrestricted Physical Therapy Assistant license in the state of North Carolina
- A valid driver's license, auto insurance, and reliable transportation
- Current CPR certification
