Job Summary
Provide in-home physical therapy visits and complete evaluations, reassessments, and fall risk assessments for seniors. Document accurately and communicate progress with the care team while maintaining a flexible, per diem schedule on O'ahu. Candidates must hold a current Hawaii license, possess one year of clinical experience, and have a valid driver's license with reliable transportation. This role supports our mission to help patients stay safe and independent through quality, community-based therapy.
Required Qualifications
