Home Health PT

Assess and evaluate therapeutic, rehabilitative, and functional status while examining the home environment for muscle strength, mobility, gait, and range of motion. Initiate physical therapy programs and instruct family members or caregivers on specific phases and goals of treatment. Identify needs for additional home health services and refer patients as necessary. Participate in case conferences to develop total plans of care.
