Home Health PT Physical Therapist Full Time Osceola County

Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current level of function. Administer, evaluate, and interpret tests to determine functional goals, then provide written evaluations and plans of care defining needs, progress, and expected outcomes. Deliver physical therapy services complying with prescribed physician orders while maintaining discharge plans and educating patients and caretakers on home exercise routines. Collaborate with physicians, nurses, and families; supervise PTAs and HHAs; and complete OASIS assessments where permitted. Document client performance to ensure no technical or clinical denials and maintain effective communication across the healthcare team. Full-time weekly visits required in Osceola County.
