Home Health PT Physical Therapist Full time

Assess, teach, and deliver physical therapy interventions directed by the physician within the home health plan of care. Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels while administering tests to determine goals and outcomes. Provide written evaluations, modify treatment plans as needed, and complete OASIS assessments to ensure compliance with physician orders and payer documentation standards. Collaborate with interdisciplinary team members, physicians, and caregivers to optimize patient well-being, supervise personnel such as PTAs and HHAs, and discharge patients with home exercise plans. Full-time Monday through Friday visits in Manatee County.
