Job Summary
Home Health Physical Therapist (PT) needed for full-time, in-home patient care in Kenansville, NC. Responsibilities include initial and ongoing mobility/strength/coordination assessments, developing and implementing a plan of care, documentation of progress and outcomes, coordinating with the healthcare team, discharge planning, and completing OASIS assessments where allowed. Requires state PT licensure, CPR, valid driver's license and auto insurance; preferred experience in home health and OASIS documentation.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR, Drivers License and Proof of Automobile Insurance
