Job Summary
In-home Physical Therapy role providing comprehensive PT services to patients in their own homes. Responsibilities include performing initial and ongoing mobility/strength assessments, delivering therapy per physician orders, documenting care plans and progress, conducting OASIS assessments where allowed, discharge planning, coordinating with healthcare teams and family, supervising PTA/OTA/HHA staff as applicable, and ensuring compliance with licensure and policy standards. Full-time weekly visits in Osceola County with on-call weekend rotation; benefits include medical/dental/vision, 401(k), paid time off, CEUs, tuition assistance, and employee support programs.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR, Drivers License, and Proof of Automobile Insurance
