Home Health PT Physical Therapist Full Time

Perform initial and ongoing assessments to evaluate mobility, coordination, strength, and current function. Administer, evaluate, and interpret tests to determine functional goals and outcomes. Provide written evaluations and plans of care defining physical therapy needs, goals, progress, and expected outcomes while delivering services to comply with prescribed physician orders. Maintain plans of care through timely documentation, modifying goals and treatment as needed, and determining discharge care plans. Collaborate with the healthcare team, physicians, and caretakers to educate patients on home exercise plans and supervise other personnel where applicable. Complete OASIS assessments where allowed by regulation and document effectively to prevent technical or clinical denials. Full-time weekly visits in Seminole County.
