Home Health PT Physical Therapist Full Time

Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and current functional levels. Administer, evaluate, and interpret tests to define functional goals and outcomes, then provide written evaluations and plans of care. Deliver physical therapy services per physician orders while maintaining discharge plans and educating patients and caretakers on home exercise regimens. Collaborate with physicians, nurses, and family members, supervise PTAs and HHAs, and complete OASIS assessments. Document client performance and modify treatment as needed to ensure accurate payer reviews and zero clinical denials. Maintain effective communication across the healthcare team and serve as a clinical resource consultant.
