Job Summary
Conduct initial and ongoing assessments to evaluate mobility, coordination, strength, and functional levels. Deliver physical therapy services per physician orders while administering, evaluating, and interpreting tests to set functional goals. Maintain written plans of care, document client performance, and modify treatment as needed to ensure timely discharge planning and education of caretakers. Collaborate with physicians, nurses, and family members, supervise PTAs and HHAs, and complete OASIS assessments where permitted. Ensure all documentation complies with policy to prevent technical or clinical denials.
Required Qualifications
- Degree in Physical Therapy from an accredited university/program
- Current, unrestricted state license as a Physical Therapist in the state of practice
- Valid CPR
- Required vaccinations and testing
Desired Qualifications
- Experience in Home Health Therapy
- Completion of OASIS documentation
