Home Health PT/OT Lakeland

Perform Start of Care (SOC), recertifications, evaluations, and routine visits in the home. Develop and implement individualized care plans while completing accurate and timely OASIS documentation. Educate patients and caregivers on treatment plans and safety, and coordinate care with interdisciplinary team members. Ensure compliance with all state and agency regulations. Applicants must hold an active, unrestricted Florida PT or OT license with at least one year of recent home health experience.
