Home Health Psych Registered Nurse RN PRN

Deliver individualized, one-on-one nursing care in the home to promote patient recovery, safety, and independence. Conduct comprehensive in-home assessments evaluating physical, emotional, and environmental needs, then translate findings into patient-centered care plans. Provide skilled interventions including wound care, infusions, medication management, and chronic disease management tailored to each patient's goals. Educate patients and caregivers on symptom and medication management while monitoring progress to prevent avoidable hospitalizations. Coordinate care across providers and complete accurate documentation supporting quality outcomes. Requires an active RN license with one year of clinical experience, CPR certification, and a valid driver's license.
